The Jade Terrace Longevity Register Confession FULL NAME IS

Supreme Jade Terrace’s Precious Book of Increasing Blessings and Ritual of Prolonging Life

Author unknown. It appears to have been compiled between the Tang Dynasty and Song Dynasty.
Consisting of one scroll, it is included in The Daozang (Taoist Canon), under the Dongshen Section, Ritual Category.

This work lists the honorific titles of 150 Heavenly Worthies and four True Immortals. It states that with sincere devotion, surrender, and reverence to these Heavenly Worthies and True Immortals, one can protect their body, dispel misfortunes, and attain longevity with a long and healthy life.
